Creative Age is for people living with dementia at home and their supporters/family/carers.
To apply to to join our sessions you can download an application form or complete the online application below.

Once completed please you can send your form via email or post.

After your application has been received, Rachel will phone you. We will then arrange a visit so you can meet one of the team, and you can then find out more about what to expect at Creative Age. You can also phone Rachel on: 01242 269803
